The Connecticut State Medical Society, with funding from the Connecticut Health Foundation, and in collaboration with Community Mental Health Affiliates, has developed a training guide and toolkit to help healthcare professionals collect sensitive information to identify and address health disparities based on cumulative social disadvantage.
